Zip-IT for Families of Multiples is a program that helps families of multiples (twins, triplets, etc.) in need of medical/financial assistance. Families of Multiples will be referred to as (FOM) at times. Select the “More for FOM” link for more detailed information on this program.

To provide financial assistance to families in hardship within our local communities and aboard. Helping families that really need help, one family at a time.

Application Form

Key Benefits

• Families receive financial help for medical bills for their children
• The medical entity is paid on behalf of the applicant
• The family receives a tax relief due to the medical entity being paid which is not counted as additional income

Program Requirements

• All applicant’s must complete the Family application form and submit the following information:
• Applicant must sign and return Authorization Release Form
• Applicant must show proof of a real need for assistance
• Applicant must provide proof of income (last W2, 1099, or prior year tax return)
• Applicant must list tax bracket of year prior to application
• Applicant must list and show proof of insurance coverage for child (Medicare, Peach care, etc.)
• Applicant must list and show other sources of income (e.g. child support, income from other agencies, etc.)
